Here you will find my new patient intake form that you can down load, fill out and bring to your first appointment.

Don't worry about questions you may have, just write them down and bring them in with you and we will discuss them at that time.

Also, be sure to list each of the pharmaceutical drugs you are taking, please use complete names and dosages.

Likewise list any other supplements you are taking.

It is often helpful for you to make a list of your medications and supplements on your computer where you can save them for future reference or to help you remember what you are taking and when you need to take it.

Please bring your list, handwritten or word processor created and produced with you on your first visit. This is very helpful.

Also, please read and sign the included Informed Consent For Acupuncture Form and bring that with you on your first office visit also.

It is important for you to wear comfortable appropriate clothing. By this I mean bring or wear shorts and a sleeveless top or tee shirt. I use a lot of points on the arms and legs and this will enable me to more easily and quickly access them. We have a large bathroom for changing clothing if you need to change into your shorts and shirt.
